Try unplugging the MAF and see what happens.

The MAF is a frequency signal so freeze frame voltage data only tells 1/2 the story. The other reasons behind that code are things you already checked into even though vacuum leaks are very tricky to find and can sometimes be missed even after multiple attempt to look for them, but unplugging the MAF takes 3 seconds and is a good way to rule out a dirty/faulty reason behind that code.

Document ID: 561061 in the service manual ^^^^^ is p0171 specific and could help with some other checks.

Just go to the top of this page on the right side of the header, click on service manual, then "Find a document...." and paste that ID number in there and it will run you through a whole list of checks. I'd still start by unplugging the MAF and see if the lean condition goes away, since that's the easiest place to start.

Score for having a lead to the solution, but boo for kidney surgery. Hope it went well.
